Contact information

If you are sending a request for quotation, you are not required to enter all fields. Your name and your e-mail address will suffice. If you are placing an order we would be grateful if you would supply as much relevant information as possible.

Name

Always required.

Telephone number

Always required.

Purchase order number

Only if required by your organisation.

E-mail address

Not required, but appreciated.

VAT number

Only required if your organisation is located within the EU (except Sweden). If you don't have the VAT number of your organisation available at the moment, you may leave this field blank and supply the number later on.

Delivery address

Always required.

Invoice address

Only state this if it differs from the delivery address.

Product

 There are currently two RetroSys™ products; the RetroSys™ RT Activity Kit and the RetroSys™ C-type RT Activity Kit.

Cost and currency

Choose whether the invoice should be stated in Euro or US Dollar.

Number of kits

Specify how many kits of each product you would like to purchase.
